DIABETES EDUCATION
Provides diabetes education and support to children, adolescents, and adults in the community.
Philosophy
Hanover Hospital advocates the provision of diabetes patient education as integral to the care and management of the person with diabetes.
Staff
Diabetes counseling is provided by registered nurses and a dietitian that are specialized in diabetes management and are Certified Diabetes Educators.
Referrals
Health care providers and individuals with diabetes may contact the Diabetes Educators to schedule an appointment or register for class by calling 717-633-2118. All individuals must have a completed referral form from their physician prior to scheduling an appointment. (Diabetes Outpatient Education Physician Referral)
Services:
Individual Outpatient Consultations
An individual appointment includes a needs assessment, the development of individual goals, discussion of the disease process, self-management training and follow-up to assist individuals and their families in dealing with lifestyle changes related to diabetes management.
Diabetes Group Classes
The Diabetes Puzzle, "Putting It All Together" is a seven-part, 10-hour program designed to help individuals manage and monitor their diabetes. A different aspect of self-care will featured at each session tailored to individual needs. Topics include; meal planning, exercise, medications, blood glucose monitoring, record keeping, sick days, food shopping, dining out, stress, travel, family involvement, and complications. This course is recommended for all newly diagnosed individuals.
Classes are offered Tuesday evenings 6:30-8:30 p.m. or Thursday mornings 9:30-11:30 a.m. For a complete schedule of class dates and times, please call the Diabetes Education Department at 717-633-2118.
Individual instruction is provided for individuals new to insulin, and other injectable medications.
This program provide nutrition counseling, discussion of gestational diabetes and monitoring equipment necessary for the management of gestational diabetes to achieve treatment goals.
Individual instruction is provided for individuals in monitoring their blood sugar. Blood sugar goals are discussed and participants are given a blood glucose meter that best suits theirs needs and meets insurance criteria.
Individualized services are available for children and adolescents, visually impaired, the learning disabled, hearing impaired and non-English speaking participants.
Nutrition therapy is provided by a registered dietitian with a special interest in diabetes to assist individuals in their meal planning needs, and nutritional requirements.
Insurance reimbursement may be available from Medicare, Blue Cross, Keystone and other insurance carriers. The cost of the group class includes a one hour pre-class assessment, 9 hours of group instruction, and follow-up at: one month, six month and one year. Family members or support persons are invited to attend at no additional charges
Diabetes Table Talk is a free program offered six times per year (April, May, June, September, October, and November) to provide individuals with with information on diabetes management and support for individuals with diabetes and their families.
