PREPARING FOR DISCHARGE
The Day You Leave
Discharge planning begins on the day of your admission. Your physician, and
your rehabilitation team will work with you to meet goals that will lead to
your discharge from the rehabilitation center.
Discharge does not necessarily mean that you have reached all of your goals. Your recovery continues beyond discharge. The rehabilitation team will provide you with choices, if needed, regarding home health care, outpatient services, and assistive equipment.
Discharge instructions will be given to you and your family on the day you leave. If you are going to another facility, detailed records will accompany you.
Patient Satisfaction Survey
At the time of discharge, you will be given a confidential Patient Satisfaction
Survey. We ask that you complete this survey together with your family.
We are very interested in your assessment of our program and value your responses to the survey. Please feel free to write any comments on the last page of the survey. Our staff does not read the surveys, however they are provided with the results.
Your thoughts about your experience as a patient with us is very important as we continually strive to offer the highest quality patient care possible.
We will ask you if we may contact you 90-120 days after your discharge to check your progress. This information will be used to improve our service.
Who to Contact if You Have Questions
Whatever the problem, concern or question, feel free to consult your nurse,
therapist, social worker or physician. We are here for you and your family and
want to make your stay as pleasant as possible.
After discharge, you are not forgotten. Team members can be a useful resource for you. Please call with any questions you have once you are home. You can reach us by calling (717) 646-6868.
