Full Time Benefits - Health and Prescription Plan

Who pays for it?
HHCP and you.

When are you eligible?
First of the month following the date of hire.

Description of Benefit
The health plan is administered by South Central Preferred, a preferred provider organization.  Coverage includes hospitalization, surgery, x-rays, lab work, office visits, and annual exams. Participants in the medical plan are automatically enrolled in the Prescription Drug program. This service is provided through Express Scripts, Inc. which has a network of pharmacies nationwide, including the Hanover Apothecary.

Full Time Benefits - Dental Insurance

Who pays for it?
HHCP and you.


When are you eligible?
First of the month following the date of hire.

Description of Benefit
Dental insurance is provided through United Concordia (UCCI)and includes coverage for diagnostic, preventative, basic/major restorative and orthodontia. UCCI is network-based. When services are obtained through a network dentist, that provider accepts UCCI's allowance as payment in full. (The employee is responsible for any deductible or co-insurance amounts.)

Vision insurance is provided by Vision Benefits of America.  VBA maintains a network of over 15,000 providers nationwide. The plan includes coverage for exams and materials.

Description of Benefit
Health care and dependent care accounts that allow you to set aside money for unreimbursed health care expenses and/or dependent expenses free from Federal and FICA taxes, resulting in savings for you and your family.

Full Time Benefits - Extended Illness Bank (EIB)

Who pays for it?
HHCP.

When are you eligible?
The first pay period following 6 months of continuous employment.

Description of Benefit
Annual accrual totals six (8-hour) days per year.  Maximum accrual of 200 hours. EIB may be accessed after using one workweek of PTO.

Full Time Benefits - Short Term Disability

Who pays for it?
HHCP

When are you eligible?
After 180 days of service.

Description of Benefit
In the event of a non-occupational injury or illness, benefits in the amount of 60% of base earnings will be paid beginning after the 1st month of disability up to the 150th day of disability.

Full Time Benefits - Long Term Disability

After 180 days of service.

Description of Benefit
Applicable only if employment status is 30 or more hours per week. Contigent upon acceptance of the claim by the LTD insurance carrier, benefits in the amount of 60% of base earnings will be paid after the 150th day of disability due to a non-work related illness or injury.

Full Time Benefits - Paid Time Off

HHCP.

When are you eligible?
Accrues from date of hire.  Employee has access to PTO after successful completion of 90-day initial review period.

Description of Benefit
Paid time off is accrued based on eligible worked/paid hours, up to 40 hours per week. The amount available can be computed by multiplying eligible hours by the appropriate accrual factor.

Description of Benefit
The EAP helps employees deal with problems that might adversely impact their work performance, health and well being.  Employees and immediate family members are each eligible for three free counseling sessions per contract year.

Description of Benefit
A voluntary payroll deduction, taken from your pay before federal income tax. All contributions are invested in options you choose as offered by Lincoln Financial Group.

Full Time Benefits - Defined Contribution Retirement Plan

Who pays for it?
HHCP.

When are you eligible?
Completion of 1 year of service with at least 1,000 hours of service and attainment of age 21.

Description of Benefit
Defined contribution plan with an HHCP discretionary contribution of 3% of employee's base salary. In addition, HHCP will match 50% of your voluntary contributions to the Tax Sheltered Annuity (TSA) up to an additional maximum employer contribution of 2%.

Full Time - Voluntary Life Insurance

Who pays for it?

When are you eligible?
First of the month following the date of hire.

Description of Benefit
Employee supplemental life can be purchased in increments of $10,000.  The maximum is the lesser of five times annual earnings or $500,000.  Spouse coverage can also be purchased in increments of $5,000 to a maximum of $50,000.  Child coverage can be purchased in increments of $5,000 to a maximum of $10,000.  Evidence of insurability may be required.

Reimbursement for tuition of approved courses as follows:

  • Grade of A or B - 100%
  • Grade of C - 75%
  • Grade below C - 0%

*Reimbursement up to $7,000.00 available for selected degree programs.
