Hanover Medical Group physician offices provide patient centered care from a care team who knows you and is dedicated to meeting your healthcare needs.  Eight of our primary care locations have achieved Patient Centered Medical Home (PCMH) Recognition, Level III.

We encourage you to participate in decisions about your healthcare and assist you in navigating the healthcare system. We strive to keep you healthy and work together with you to set goals and remind you to complete preventative care services.

Your primary care provider is here to coordinate all your healthcare needs and offers you the best access to coordinated care between hospitals and specialists. Our primary care providers offer same day appointments and extended hours. Our Express Care offices are open every day from 8 a.m. to 8 p.m.

You also have access to our Patient Portal, which is a secure portal that allows you to view your healthcare record, request an appointment or send a message or question to the office anytime of the day or night.

Patient-Centered Medical Home (PCMH)

The National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the health care system. Research shows that medical homes can lead to higher quality and lower costs, and can improve patient and provider reported experiences of care.

To earn recognition, which is valid for three years, practices must demonstrate the ability to meet the program’s key elements, embodying characteristics of the medical home. NCQA standards align with the joint principles of the Patient-Centered Medical Home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association.

We would like to congratulate Dr. Vipul Shah and the Hanover Medical Group (HMG) Grandview Family and Adult Medicine Team for being the first HMG office to achieve Patient Centered Medical Home Recognition, Level III Distinction from the National Committee for Quality Assurance. Dr. Shah and his team were the pilot site for the first of our HMG primary care offices to achieve the Patient Centered Medical Home (PCMH) recognition.

A total of eight HMG primary care locations were submitted for PCMH recognition, and at this time we have received confirmation that all eight, including the Grandview, Littlestown, Hanover, McSherrystown, Spring Grove, South Hanover, New Oxford, and Adult Medicine Multi-Specialty Group locations have also achieved recognition.

Hanover Medical Group (HMG) policies

The following policies apply to all Hanover Medical Group (HMG) offices:

  • Refills: Refills of existing prescriptions will be filled within two business days of receipt of the request.  Request for prescription refills cannot be granted after 2 p.m. on Fridays.  Please note that you may need to be seen by the physician prior to a full refill being given.
  • Refill of controlled drugs: Refills of controlled drug prescriptions (narcotics, sedatives) will be limited to those prescribed by your provider.
  • Form Completion: Forms must be completed within seven business days of receipt of the request.  A fee for this service does apply to some forms.  Please inquire regarding fees when you request the completion of a form.
  • Payment: Payment is expected at the time of service. Please call your provider's office if you are unsure if they accept your insurance.  Should you have additional insurance carrier and/or billing questions, please contact Physician Billing Services at 877-205-3879.
